1/23/2024 0 Comments Long term symptoms of insomniaThis view was also noted by the European Guideline for the Diagnosis and Treatment of Insomnia, which was endorsed by the World Sleep Society. harms of the long-term use of pharmacologic therapies for chronic insomnia. The recommendation by the ACP Clinical Practice Guideline on the Management of Chronic Insomnia Disorder in Adults that medication use should be limited to 4–5 weeks derives from the assessment that the evidence is insufficient to adequately weigh the benefits vs. The use of BZDs, and in particular long-term use, is not recommended in older adults due to the increased risk for these adverse events. BZDs, for example, have been associated with the potential risk of falls and cognitive impairment, as well as the potential of abuse and dependence. Guidelines recommend a shared decision-making model when selecting pharmacologic agents for insomnia, with consideration of the types of patient-reported symptoms (difficulty falling asleep, staying asleep, early morning awakening), medical and psychiatric co-morbidities, concurrent pharmacotherapy, and symptom duration when weighing the risks and benefits of specific medication classes and duration of use. Data from multiple randomized controlled trials support the efficacy of several approved pharmacological treatments including several different drug classes, such as benzodiazepine (BZD) and non-benzodiazepine (non-BZD) GABA-A modulators, dual orexin receptor antagonists (DORAs), melatonin receptor agonists and histamine antagonists. While cognitive behavioral therapy for insomnia (CBT-I) is recommended as first-line therapy, pharmacologic therapy is commonly used in the management strategy for treating insomnia. Insomnia is also bi-directionally linked to mood disorders, in that insomnia is a risk factor for depression and depression has been shown to predict the persistence of insomnia. Women experience higher rates of insomnia compared to men and the prevalence of insomnia increases in older populations. By the time most patients with insomnia present for treatment, sleep has been disrupted for a duration of at least 3 months and thus meet the criteria for chronic insomnia disorder. The clinical diagnostic criteria defining insomnia disorder include not only symptoms of difficulty falling asleep, staying asleep and/or earlier awakening than desired for at least three times per week, but also the consequent daytime functional impairments. Insomnia is among the most common medical conditions worldwide, affecting 15–30% of the general population. Thus, an evaluation of evidence supporting the long-term safety and efficacy of newer non-benzodiazepine hypnotics is timely and should be considered in practice recommendations for the duration of pharmacologic treatment of chronic insomnia. For eszopiclone, doxepin, ramelteon and the newer class of dual orexin receptor antagonists, the FDA label does not specify that their use should be of a limited duration. After discussion of the literature, the panel unanimously agreed that some classes of insomnia medications, such as non-benzodiazepines hypnotics, have been shown to be effective and safe for long-term use in the appropriate clinical setting. Survey respondents revealed a wide range of opinions regarding the appropriateness of using the US Food and Drug Administration (FDA)-approved medications for the treatment of insomnia lasting more than 3 weeks. The panelists’ assessment was also compared to findings from a national survey of practicing physicians, psychiatrists, and sleep specialists. A panel of sleep experts conducted a clinical appraisal regarding the use of insomnia medications, as it relates to the evidence supporting the focus statement, “No insomnia medication should be used on a daily basis for durations longer than 3 weeks at a time”. While evidence supports the benefits of medications for the treatment of chronic insomnia, there is ongoing debate regarding their appropriate duration of use.

1/23/2024 0 Comments Green stack socksAre GPs more likely to lose their jobs from use of the green socks clause at this time or from using socks given at Christmas? A study could also look at other dates such as St Patrick’s Day celebrations on 17 March each year where green accessories, perhaps socks as well, are possibly putting extra strains on GP partnerships. The sock colour green is very popular at Christmas and is a common colour on novelty Christmas socks. As GPs conduct home visits the need to travel outside in the cold increases the likelihood for wearing warm socks. Is there seasonal variation in sock-wearing behaviours that may put GP partners at higher risk in certain seasons? Most results were submitted in the winter months when people tend to opt for warmer clothing. However, further studies could be directed at looking at actual GP partnership agreements and, for those with the green socks clause in it, examining the sock-wearing behaviour and knowledge of said clause among the GPs at those practices.įinally, a separate subtopic has come out of this study that GP researchers could look into in the future. However, it may not remove the risk of similar such clauses existing. Perhaps male GPs prefer green generally or there may be a narrower choice of sock colours for them? Or some male GPs may be aware of such a clause and feel that wearing well-tailored trousers can hide such coloured socks and avert their risk?Īs so few partners were aware of the clause, it’s unlikely that many partnership agreements will contain it. This possible gender difference in the colour of socks being worn by males and GPs could be further examined. Therefore, 9.35% of the responders are potentially at risk of expulsion due to their green sock-wearing habit (if they wore the same colour socks while working as a GP partner at a practice that had a green socks clause, that is). This included 90% of GP trainees, 77% of locums, 68% of salaried, and 43% of partners. Furthermore, how many GPs are unaware of the green socks clause? The answer is that 57% of the responders were not aware. Of these, 30% were female and 70% were male. This included 15 partners, 11 locums, 3 GP trainees, and 1 salaried. So how many GPs wear green socks? It was found that 30 GPs (9.35%) wore green socks on their most recent day at work. Within these respondents, 52% were female and 48% male, mirroring the gender statistics of the GP register where 51.7% are female and 48.3% male. This represented 197 partners, 44 salaried, 50 locums, and 30 registrars. An online questionnaire was answered by 321 GPs across the UK regarding their sock or tights colour on their previous working day. However, we have attempted to address this slightly by undertaking a recent observational study last year. There has been no research into how aware GPs are of this clause, nor of their sock-wearing habits. This can be helpful when the relationship between partners has irretrievably broken down 4 and can make dissolving partnerships easier and less protracted. 1 – 3 It was developed from similar clauses used in legal partnerships whereby a partner can be expelled without reference to their work but is instead described as due to their wearing of green socks 2, 4 – 5. An increasing number of GP partnership contracts contain this legal caveat known as ‘the Green Sock Clause’. This is a legitimate and legal approach within some GP partnership agreements, as a way of removing a GP partner. Simply wearing a particular colour of socks could lead to a GP partner losing their job. 1/23/2024 0 Comments Mini projector for iphone walmartThe cylindrical design captivated us upon unboxing, and then the setup and super-intuitive interface screen won us over. Read our BenQ HT2050A review. The Nebula by Anker Capsule Max is slightly bigger than a soda can (OK, maybe a tall boy), but it packs a tremendous punch. Its picture is also much better in pretty much every way, so if you're looking for a projector for use around the home, or maybe backyard, and you don't need battery power, it's worth considering. Read our Anker Nebula Cosmos Laser 4K Projector review.īenQ HT2050A: The HT2050 isn't technically a portable projector at all, but it is compact and roughly the same price as one of the more expensive "portable" options. It's nice and bright, but the other downsides - lack of zoom, average overall image quality and a steep price for what you get - keep it off this list. There's also no battery so you'll need to plug it in. Read our Xiaomi Mi Smart Projector 2 review.Īnker Nebula Cosmos Laser 4K: Since it has a handle and a compact size, the Cosmos Laser 4K is technically portable, but it's hardly mini at more than 10 pounds. It's relatively dim, especially for the price, and it lacks both a built-in battery and compatibility with a USB power source, so it's not truly portable. We appreciated the compact size, 1080p resolution and built-in Android TV streaming, but the cons outweigh the pros. Xiaomi Mi Smart Projector 2: Another relatively expensive small projector we didn't love, the Xiaomi at least has a gorgeous, Apple-like design going for it. Worse, its price is a good 50% higher than it should be based on how it looks and performs. However, it lacks a battery and its performance is average, at best. Its built-in streaming is far better implemented than most portable projectors. About the size of a Bluetooth speaker, and in fact can double as one, the Freestyle can pivot on its stand to project an image at any height on walls and even the ceiling. Samsung Freestyle: The small, cylindrical Freestyle is an interesting idea, but it comes up short. The battery doesn't last as long as the AAXA either although it is a bit cheaper. It's not particularly bright, however, nor does it have built-in apps. LG CineBeam PH30N: The LG PH30N is tiny even compared to other portable projectors, even smaller than the AAXA. However, it's dimmer than the Xgimi Halo Plus for similar money. You can even use it as a Bluetooth speaker. Read our Anker Nebula Solar Portable review.īenQ GS50: The GS50 is roughly the same size as the Xgimi Halo Plus, and is also 1080p and has a built-in battery. If you want a sleek, budget-friendly portable with 1080p and plan on keeping the image on the small side, however, this is a solid choice. Its main disadvantage compared to others on this list is its relatively dim image, which means it can't project as large a picture and still look good. But if you want something compact, portable and maybe battery-powered, here are our top picks for the best mini projector.Īnker Nebula Solar Portable: We didn't like the Solar Portable as much as the BenQ GS50, Xgimi Halo Plus or even its sibling, the Mars II Pro. If you're never going to be far from an outlet, a standard projector will get you a much bigger, brighter and better image for similar money. Another is that most portable projectors, especially the cheapest ones, often have lower resolution than their larger counterparts, especially 4K projectors. That means they can't project as large an image as the big guys. One is that many are relatively dim, lacking the brightness of a traditional home theater projector. Portable projectors do come with some drawbacks, though. Many mini projectors can give you access to Netflix, Amazon Prime Video, Disney Plus, Hulu and other streaming services without you having to connect a streaming device or laptop. They also offer a handful of connectivity options, including Wi-Fi, HDMI and Bluetooth, and typically run on batteries. Whether you project onto a blank wall or a projector screen, you can watch your favorite shows and movies without getting weighed down. The best portable projectors have perfectly good picture quality and are easy to set up in any room of your house or in the great outdoors (or at least your great outdoors). I didn't know that projectors could be amazing for outdoor movie sessions until I tried one. 1/23/2024 0 Comments Lama surya das divorceAfter much trial and error, I have come up with my own practice for regulating strong emotions and being patient and more authentically responsive through these six steps to mindful anger management and intentional responsiveness. It is important to realize that anger has its own function, intelligence and logic and so we should not entirely try to eradicate it. Discovering methods to deal with these challenging emotions is essential in leading a healthy well-balanced, harmonious life. I have found that fear, anger and irritation are like an affliction, and a serious impediment to open communication and healthy relationships of all kinds. “The whole universe is my body, all beings my heart & mind.” Considering trends, problems, challenges and opportunities we face, I believe we must ask ourselves and each other: “What can we do to contribute to a better world and planet? What can we do individually and collectively toward alleviating suffering and edifying and awakening the world through our noble-hearted and compassionate, all-inclusive bodhichitta (innate enlightened mind), thru love in action, for the benefit of one and all?Īs a tantric Tibetan song of enlightenment goes: It’s time to open our wisdom eye and good heart, and grok this world, clear and open. The future begins now, starting with one step. I know now that we can’t just ask what needs changing without sincerely striving to know and transform ourselves. And our broken social systems also need transformation. We do need genuine change and transformation, each and all of us. Unfortunately, we seem unable to recognize the interdependence of all.Ī sage said: “The light by which we see is the one by which we are seen.” You can see this at many levels, human and divine. Recognizing our interconnectedness and collective interdependence allows us to appreciate, respect and accept our undeniable responsibility to protect all the flora and fauna of this earth, and all the habitats, oceans and rivers too. I find it by going mindfully outside, interbeing with the beauty and richness of nature, and observing directly the inseparable unity of doing and being in moments of inter-meditation, co-meditating with water, sky, wind and trees and thru the inseparable unity of contemplation and action, faith and deeds, on the path of awakened living. Today we celebrate Earth Day, and I ask you: What may inspire, motivate and sustain your feelings of connection and universal responsibility, intentional altruistic actions, and the recognition that we must move from me to we if we are to survive and flourish on this endangered planet? This is based on the interwoven interdependence of all things–all of us, and all creatures great and small.
